"One break was enough, 5-4, 6-5, both sets", Djokovic said in his post-match interview.
Djokovic could meet Rafael Nadal in the semi-finals.
Fognini twice led by a break in the first set, and served for the set at 5-4, but he faltered at the line and went on to lose the set on a tie break.
Djokovic defeated Spanish veteran Feliciano Lopez 6-4, 7-5, while Nadal eased past Nick Kyrgios of Australia 6-3, 6-1. Furthermore, Ferrer is a shell of his former self, and the former world number four recorded a confidence-boosting, 6-4, 6-3 victory over the Spaniard, advancing to his fifth consecutive quarterfinal in Madrid.
In the women's draw, Eugenie Bouchard of Canada advanced to the quarterfinals after top-seeded Angelique Kerber retired with a left thigh injury in the second set.
A finalist in Madrid in 2014, Nishikori lost his last 10 matches against Djokovic, including in the semifinals in Madrid past year. In 20 games this year the Serb scored 16 victories and conceded 4 times, for comparison in the last three seasons he suffered 8, 6 and 9 defeats, winning more than 60 times.
